Some Questions from a returning player

Hi all,
I'm returning to the game after 4 years of this and that. I just got 2113 and after looking over everything there, in the 2112 ringside companion, and in past booklets, I have some questions. I appreciate any help you can give me.
1. In a coffin match, does the wrestler that gets 10 tokens automatically get put in the coffin, or does he have to be thrown in according to the chart after getting 10 tokens?
2. Does a ladder match end in a pin roll or does the match only end according to the chart with the wrestler adding 1 to his pin if pin his rolled?
3. In a tables match, do you have to roll on the chart and have 2 tables broken on the chart to lose? Does a pin count as a loss or is it ignored? It says 2 tables have to be broken for the match to end, but then it says, "If both tables are broken and there isn't a winner yet..." Were they referring to one opponent going through each table?
4. I lost my 4 Weapons of Destruction rules, but I still have the chart in my Ringside Companion. I just want to know how the match ends. I thought it was 1st to get 10 tokens loses but I wasn't sure.
5. According to Avalanche's card, you add +3 to each of the 2 pin rolls. My question is, in any match after you hit your finish, if the opponent kicks out, does he keep the + from the finish or do they lose the finisher bonus and simply gain the +1 fatigue point? I always kept the finisher bonus but after reading Avalanch's card, I wasn't sure. If Wolf (2087) uses his finisher (+3) on Star Warrior (pin 2 from 2087) and SW kicks out, is his new pin rating 5 (2+3 from finisher) or 3 (2+ 1 fatigue point)?
6. Last question: Torture Chamber Matches end when 1 wrestler gets pinned 3 times (can't continue). What happens after each of the first 2 pins? Does the attacker continue on level 3 offense?
