For the avatars, just use the URL listed above the image in the appropriate spot on your profile. Set the height and width to 100 pixels each.

For the banners, put this code into your signature:

Obviously replacing bannername with the banner of your choice. That should do it!

No Freak banner ever, I'm afraid. My artistic talent stops somewhere short of extending those cage bars across the entire page. The avatar was the best I could do there.
