Chavo becoming Kerwin White has to rank up there somewhere. That was just plain bad. I wonder how long that was supposed to last?

Terry Taylor's career might have been ruined by the Red Rooster gimmick, but One Man Gang becoming an "African" was far worse. The fact that they had to put a map of Africa on the back of his tights showed you how weak the whole idea was. This would never fly today.

Today, there is nothing funnier than popping in that old school VHS tape from SNME and seeing Slick and Akeem dancing away in the ring while Bossman looked like he was disgusted. Jive So Bro!

I gotta go with Smash to Repoman. He was a tough SOB as Smash but repo was weak, dumb, and ... well you know the rest.

Earthquake/Avalanche (they were the same gimmick) to Shark was terrible. Earthquake was one of the great big men of all time, and became a pathetic joke when he became the Shark.
Red Rooster is a close second. As others have posted, Taylor was a great cocky heel and a solid worker. He could have done much better than that.
I loved Akeem. The difference for me is that he was intended to be a comedy gimmick. OMG was just a fat unathletic guy - Akeem and Slick were hilarious to watch.
Loved Demolition, but it's hard to knock Repo Man when Darsow himself says that he had a blast playing the character.
Beefcake took a step down when he became the Barber IMO, and kept taking steps down. It's hard to know what his worst transition was - Butcher to Man With No Name, Man With No Name to Zodiac, or Zodiac to Booty Man.

I voted for One Man Gang to Akeem.
I didn't care for Taylor no matter who he was. Beefcake never stayed in one gimmick long enough for me to care about any of them. Earthquake/Avalanche changing to Shark sucked mostly for him since he changed his tatoo for a gimmick that didn't last. Him playing Golga of The Oddities was cool...but I liked The Oddities. And how could Vinnie Vegas to Oz be the worst change when both gimmicked sucked so damn baddly?!
