Since the new mail order forms don't include the SE cards, would it be okay to list the ones you want to order on a separate piece of paper (catalog # included)?

Yes you can write what you want on a seperate peace of paper
I do it since i don't always have an order form

You could always just order from the online store if you have a credit card or debit card.
